Cord blood leptin, insulin, C-peptide, IGF-1 and IGF-2 levels in relation to echocardiographic measurements in infants of diabetic mothers: preliminary report

Abstract
Background: One of the well-known disturbances in IDM is hypertrophic cardiomyopathy. Insulin and IGF-1, and IGF-2 are known to be important growth promoters in the variety of tissues. Increased cord levels of these factors are frequent findings in the infants of diabetic mothers (IDM). Leptin also may be involved in the increased body mass and accumulation of adipose tissue. Objective: The aim of the study was to evaluate whether there is an association between morphological heart parameters and cardiac function estimated during echocardiographic examination in infants of the diabetic mothers and cord blood leptin, insulin, C-peptide, IGF-1 and IGF-2 concentration at birth.Material/Methods: Among 28 infants of gestational diabetic mothers (16 AGA, 12 LGA) echocardiography measurements were performed according to the recommendation of American Society of Echocardiography. Venous cord blood samples were collected at delivery. Leptin, insulin, and C-peptide levels were measured using specific RIAs, while IGF-1 and IGF-2 levels by using specificELISA.Results: Echocardiography measurements were within the normal range in most of our examined IDM. IVSd hypertrophy was found in 5 IDM, 3 of them AGA and 2 LGA. In 6 (21.4 %) an increase of SF >40% was noticed; half of them was LGA. A significant positive correlation between SF% and IGF-1 and IGF-2 levels was found. No significant correlation was noticed between all analyzed echocardiography parameters and insulin, and C-peptide levels.Conclusions: There is an association between dynamic function of the left ventricle and cord blood IGF-1 and IGF-2 levels in IDM.
